This notice explains how our 30-day check-in quiz (at /pages/quiz) handles consumer health data, such as height, weight, goals, diet, activity, energy, medications and safety answers. It supplements our store Privacy Policy.

What we collect

Your quiz answers are processed only in your own web browser to show your plan on screen. They are not sent to, stored by, or shared with Formulation Makers Inc. or anyone else. Closing the page or pressing "Start over" clears them.

The page stores one small item in your browser (which of five headline versions you were shown) so you see the same version if you return. It contains no health information.

Advertising measurement

If you arrived from one of our ads, our advertising tools may record that you viewed the page, started the quiz, or finished it, along with the headline version number. No quiz answers or health information are included in those events.

We do not sell health data

We do not sell consumer health data or use your quiz answers for advertising.
